2. Please note some additional instructions for submission of form based on the Option in the Proxy Form you choose.

3. Option 1, you may click submit and your form will be returned to the Club and a copy emailed to you or click print and deliver a copy back to the Club.

4. Option 2, you may follow option 1; however, it is your responsibility to deliver your proxy to your acting Shareholder/Third Party Proxy Holder.​
